A typical day for an Intake Clinician WesleyLife at Home/Home Health:
Receive and triage home health (and backup hospice) referrals from all sources via phone and email.
Explain home health (and hospice) services to phone callers including physicians and other health care referral sources and clients, family members, and caregivers.
Ensure documentation and orders are in place; monitor required documentation ongoing.
Complete, maintain, and submit accurate and relevant clinical notes regarding intake/referral information using the electronic medical record (EMR).
Communicate with referral sources, physicians, and clients via phone and email about referral status.
Travel in all weather conditions and to all areas of the home health service area to perform co-visits or client visits as needed.
May participate in a home health on-call rotation for nights and weekends.

Your normal schedule will be 8 a.
m.
to 4:
30 pm.
Monday through Friday at our Urbandale office.
This is not a remote position, and work-from-home is not permitted.
