Essential Functions & Responsibilities

The Advanced medical care social worker is a core member of the multi-disciplinary team and provides an advanced level of bio-psychosocial and care management services to identified high risk patients. This position works collaboratively with the professional care team from multiple disciplines and across the continuum of care to develop and implement an interdisciplinary plan to provide coordinated care to improve outcomes for high risk patients with multiple comorbid conditions.
Provides bio-psychosocial-spiritual assessment of the patient to identify priority needs, strengths, patient preferences, and barriers to care access.
Provides home assessments when appropriate to complete a comprehensive assessment of the patient/family.
Active as a core member of the care team in the development, implementation and evaluation of the plan of care.
Identifies and delivers culturally competent social work interventions.to patients/families to meet identified needs.
Serves as a patient/family advocate around concerns such as patient confidentiality, informed consent, patient autonomy, and self -determination.
Qualifications/Experience
Masters degree in social work from an accredited school of Social Work and current Iowa State License at the master's level.
Minimum 3 years clinical experience in health care.
LISW preferred
Strong verbal and written communication skills.
Ability to take initiative and exercise independent judgment, decision making and problem solving expertise.
Ability to work as a collaborative team member.
Ability to understand and apply guidelines, policies and procedures.
Licensed driver in the State of Iowa with automobile insurance in accordance with state and/or organizational requirements.
